Accident reports can be obtained through our office during normal business hours (Monday - Thursday 8:00 am to 5:00 pm). You must be the subject of the accident report or the legal parent or guardian of a minor involved. If an accident is being investigated, generally accident reports will not be released until the case has been approved by a supervisor and closed. This can take a few business days up to several months depending on the case. Requests must be received in writing and proper identification is required. You may fill out a request at the police department front desk, or email a request to firstname.lastname@example.org. Email requests must include a copy of your driver’s license, your name, date of birth and detailed information about what you are requesting.
If you have a criminal case pending against you, you can obtain a copy of your arrest report through your defense attorney. If you are not represented by counsel, and your case involves misdemeanor charges, you may request a copy of your report by emailing the City Attorney's office at email@example.com, or faxing 801-796-0984. In the subject of the email, please include your Name and Prosecuting City (if applicable). In the body of the email, please include your name, date of birth, the police case number and/or citation number, the court case number if you have it, a valid email address/mailing address, and the person cited if other than yourself.

If you are requesting any other record not involving a pending criminal case, please email your request to firstname.lastname@example.org. You should expect up to 10 business days for a copy of the report to be processed.
We offer Reference Letters to citizens of American Fork or Cedar Hills. There is no charge for this service. You must come into the police department with a photo ID and fill out a request. Letters can usually be taken care of while you wait.
